Job summary

Avamere Rehab of King City in Tigard, OR is seeking a Social Services Coordinator to support residents’ psychosocial needs and coordinate safe discharge planning. The role collaborates with the Social Services Director and an interdisciplinary team to ensure comprehensive, patient-centered care.

The position requires a bachelor’s degree in public health administration, social work, or related field, plus experience with EMR systems and SNF settings.

Qualifications

• Bachelor’s degree in public health administration, social work or a related field.
• Experience with electronic medical records and computer documentation systems.
• 1 year of experience in a skilled nursing facility or similar health care setting.

Responsibilities

• Identify residents’ emotional, mental, and psychosocial needs and ensure care plans are current.
• Coordinate discharge plans and barriers to ensure safe discharge from the facility.
• Communicate with residents, families, providers, and payors on a timely basis.
• Advocate for residents and uphold their rights during care.
• Coordinate with community support services to ensure post-discharge needs are met.
• Arrange transportation and ancillary services as needed for residents.
• Lead care conferences with residents and families; monitor mood and behavior for supports.
