Qualifications

Responsibilities

• Conduct initial social service assessments including MMSE and intake for MDS.
• Review nursing charts and complete MDS within facility timelines.
• Develop written plan of care identifying resident problems and goals.
• Ensure care plans are used in daily social services and reviewed with nursing staff.
• Coordinate discharge planning and social care assessments.
• Provide information on Medicare/Medicaid and financial assistance programs.
• Collaborate with community agencies for resources.
• Maintain complete and descriptive progress notes.
