Job summary

Olde Main Post Acute seeks a Director of Social Services to lead psychosocial programs for residents and families, ensuring compassionate advocacy and coordinated care across transitions.

Required is a Bachelor's degree with LSW/CSW licensure and healthcare social work experience; master’s degree preferred. Strong regulatory knowledge and excellence in communication are essential for this in‑office leadership role.

Qualifications

• Bachelor’s degree required; master’s degree strongly preferred.
• LSW/CSW licensure required.
• Healthcare social work experience required.
• Nursing home/rehabilitation center experience preferred.
• Strong knowledge of regulations and standards governing care facilities.
• Excellent communication with residents, families, staff and agencies.
• Advocacy balanced with empathy and professionalism.

Responsibilities

• Provide leadership and oversight of social services programs for residents and families.
• Ensure services comply with federal, state, and facility regulations.
• Collaborate with interdisciplinary teams for patient-centered care planning.
• Provide counseling, support, and advocacy during admission, transitions, and end-of-life planning.
• Serve as liaison between residents, families, staff, and external agencies for coordinated care.
• Support staff addressing resident concerns and psychosocial needs with empathy.
• Assist with discharge planning, resource coordination, and referrals.
