Job summary

Butler Rehab and Healthcare Center in Independence, MO seeks a dedicated Social Services Director to lead the social services team and ensure the psycho-social well-being of residents. This role emphasizes ensuring compliant operations and coordinating pre-admission screenings to support families and residents.

The director provides counseling, oversees behavior charting, and maintains a patient-centered approach while navigating state and federal regulations in long-term care.

Qualifications

• Degree in Social Services or related field.
• Must be capable of performing the essential functions of the job.
• Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
• Possesses strong knowledge of state, federal and local regulations as they pertain to long term care.
• Possesses good communication skills and is patient and self-disciplined.

Responsibilities

• Monitor the resident's mental and psycho-social needs and provide services to meet these needs.
• Demonstrate knowledge of age-specific developmental factors in directing the overall operation of the facility's delivery of care.
• Participate in the pre-admission screening of all potential residents and provide information to residents and families during admissions.
• Coordinate and review behavior charting to ensure an effective plan of care is implemented.
• Provide therapeutic counseling to residents and family members to meet psych-social needs.
