Job summary

Maple Grove Wellness & Rehabilitation in Fenton, MO is seeking a Social Services Director to oversee the psychosocial care of residents in our skilled nursing facility. In this leadership role, you’ll support the emotional, social, and mental well-being of residents, ensuring smooth transitions and a high quality of life.

Ideal candidates hold a BSW, be a Missouri-licensed social worker, and have 1+ year of supervised social work experience in healthcare for elderly populations.

Qualifications

• Bachelor’s degree in Social Work or related field (Psychology, Sociology, Special Education, or Rehabilitation Counseling)
• Minimum of 1 year supervised social work experience in a healthcare setting with elderly populations
• SNF experience preferred
• Licensed Social Worker in Missouri (MO) is required

Responsibilities

• Complete psychosocial assessments and develop individualized care plans
• Maintain accurate, timely documentation to meet state and federal guidelines
• Provide emotional support and crisis intervention for residents and families
• Coordinate discharge planning and community referrals
• Advocate for residents’ rights and collaborate with the care team
• Educate staff and ensure compliance/survey readiness
