Job summary

Burbank Healthcare and Rehabilitation Center is seeking a Social Services Director to join our interdisciplinary management team and supervise an assistant. This role drives a climate and policies that maximize residents' independence, dignity, and quality of life in a long-term care setting.

Responsibilities include policy review, advocacy for resident rights, psychosocial assessments, discharge planning, and collaboration with staff to ensure comprehensive care and regulatory compliance.

Qualifications

• Knowledge of Long Term Care facilities
• At least 2 years of Social Services experience

Responsibilities

• Review facility policies with the interdisciplinary team to assure compliance with state and federal regulations.
• Participate in reviewing and setting policies concerning resident care and quality of life.
• Develop, maintain and utilize a listing of current community resources for residents and families.
• Advocate for resident rights and well-being; identify community changes affecting residents.
• Complete a social history and psychosocial assessment for residents.
• Coordinate discharge planning and referrals for home care services when needed.
• Document progress in meeting psychosocial needs and participate in QA/IDT meetings.
