Gallup Nursing & Rehabilitation is seeking a Transportation Driver to coordinate and provide transportation for residents to medical appointments, ensuring timely and safe arrivals under supervision of licensed nursing staff.

You will organize schedules, transport residents to programs and discharge destinations, and report any changes in condition to clinical supervisors. Other duties may be assigned as needed.

Qualifications

• Must be certified in the state of practice.
• Must have completed the State Approved Nurse Aide Training and Competency Evaluation Program; Nurse Aide must be listed on the state registry in good standing.
• Current BLS/CPR certification is required for SNFs/ALFs as applicable.

Responsabilités

• Coordinate transportation of residents to physician appointments and ensure timely arrivals.
• Transport residents to and from inpatient/outpatient programs and to/from facilities and residences.
• Develop driving schedules based on residents' medical appointments and program destinations; assist with transportation after hospital discharge.
• Provide reports and assessments of resident status during transportation and report any changes to clinical staff.
• Perform other duties as assigned.
