Job summary

Brentwood Place Two is seeking a Transportation Coordinator to organize and provide safe transport for residents to medical appointments under supervision of licensed nursing staff. The role emphasizes timely pickups, safe driving, and communication with facilities and clinical teams.

The ideal candidate will hold a valid driver’s license, be a state-certified Nurse Aide with CPR/BLS, and demonstrate strong safety awareness and compassion in elder care.

Qualifications

• Valid Driver's License for the applying state.
• Current Nurse Aide certification and listed on the State Registry.
• BLS/CPR certification as required.

Responsibilities

• Organizes transportation of residents to medical appointments based on priority of resident needs.
• Transports residents to/inpatient and outpatient programs and to and from facilities.
• Develops driving schedule based on resident appointments and program destinations; transports residents after hospital discharge.
• Reports on resident status during transport; notes any abnormal findings to clinical supervisor.
• Transports medical records for signatures to medical offices.
• Maintains safety and vehicle condition; logs mileage and routine maintenance.
