Job summary

Sevita is seeking a Direct Support Professional to provide compassionate, person-centered supports in a residential program. You will assist individuals with daily living activities, health maintenance, transportation, and community engagement.

This non-exempt, hourly role offers a wage of $15.00 per hour. Qualifications include a high school diploma or equivalent, at least six months in human services, and a valid driver's license, with an emphasis on safety and teamwork.

Qualifications

• High school diploma or equivalent preferred.
• Six months of experience in human services preferred.
• Must be 18 years of age.
• Working knowledge of computers

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
• Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
• Maintains confidentiality, respects the rights of persons being served, and follows Sevita policies and customer service standards.
• Reports any instance of abuse or neglect and incidents in accordance with policy and external requirements.
