THRIVE in Watertown, NY is seeking a full-time Apartment Program Assistant to support residents in an apartment-based program. You will provide daily living assistance, help with meals, clothing, and shelter, and ensure safe, supportive environments.

You will monitor residents' medications with adherence to physician instructions, document progress, and refer to social, medical, mental health, and vocational services.

Competenze

• High school diploma or GED required.
• Experience in mental health field preferred (developmentally delayed, geriatric, co-occurring disorders).
• Must maintain valid driver’s license and acceptable driving record.

Mansioni

• Assist residents with daily living needs including food, clothing, and shelter.
• Monitor medication self-administration and adherence to physician's instructions.
• Provide one-to-one support and organize therapeutic activities.
• Provide transportation for residents to needed services with staff assistance.
• Document case records, progress notes, and chart audits as assigned.
• Refer residents to social, rehabilitative, medical, mental health and vocational services.
