Job summary

THRIVE Wellness and Recovery in Watertown, NY seeks a Coordinated Entry Specialist to connect individuals with housing-related services. You will complete assessments, build landlord relationships, and coordinate access to supports to sustain housing stability.

Responsibilities include record-keeping, data entry, reporting, and collaboration with Jefferson DSS and other agencies. This role requires strong organization, confidentiality, and service coordination.

Qualifications

• Required: Associate’s degree in Human Services and two years Human Services experience.
• Preferred: Bachelor’s degree in Human Services and 1-year Human Services experience.

Responsibilities

• Meets with each recipient to provide support and complete Coordinated Entry Assessments.
• Reports to Coordinated Entry Lead and sends updates on all persons served.
• Reports to the supervisor all pertinent information concerning illnesses, accidents, untoward events, staff difficulties, repair needs, etc.
• Serves as coordinator of all housing services the person will receive.
• Links person to all services and supports necessary to maintain stable housing.
• Advocates on behalf of the individual to gain access to needed services and supports in the least restrictive setting.
• Maintains adequate written progress notes according to DSS and THRIVE Wellness and Recovery guidelines.
• Develops and maintains community/provider relationships; resolves problems affecting self-sufficiency.
• Develops professional relationships with landlords to maintain housing for the individual.
