Job summary

Qualifications

Responsibilities

• Assist the Social Service Director/Consultant in planning and directing social service programs.
• Meet with administration, medical and nursing staff to plan social services.
• Participate in discharge planning and development of social care plans.
• Interview residents and families in private settings.
• Refer residents/families to appropriate social service agencies when needed.
• Address emotional and social needs related to illness and admission to the facility.
