Qualifications

• Minimum of 1 year of social services or social work experience in a healthcare setting.
• SNF experience preferred.
• Medicaid in LTC or SNF settings preferred.
• Knowledge of Code Statuses preferred.

Responsibilities

• Conduct resident and family interviews to gather social histories and identify needs.
• Assist with discharge planning and coordinate community resources and services.
• Support development and implementation of individualized social service and care plans.
• Participate in resident assessments, care planning meetings, and departmental programs.
• Provide compassionate support and customer service to residents and families.
• Maintain accurate documentation in accordance with policies and regulations.
• Report occupational exposures per facility policy.
• Participate in emergency drills and the facility's preparedness plan.
• Stay current on best practices and integrate effective approaches into programs.
• Collaborate with the interdisciplinary team to promote quality resident care.
