Qualifications

• Bachelor’s degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or related field.
• Experience with seniors or disabled individuals preferred.
• Strong ethics and client confidentiality.
• Ability to collaborate with residents and partners.

Responsibilities

• Assess residents' needs to identify services that enhance quality of life and independence.
• Build connections with local service providers to access social, health, and wellness resources.
• Create and sustain educational health and wellness programs.
• Develop and maintain a directory of community resources for residents.
• Advocate for residents and liaise with community services.
• Serve as a liaison with management and external agencies providing social services.
• Provide information, referrals, and crisis intervention support.
• Monitor referrals and ensure follow-up with external agencies.
• Maintain daily and monthly records documenting service coordination.
• Perform other duties to support RHF's mission.
