You will work with the property team to develop service plans, monitor outcomes, build community partnerships, and help residents access food, housing, and utility assistance.

Qualifications

• Bachelor of Social Work, Gerontology, Psychology, Counseling; or equivalent combination of education, training and experience.
• 2-3 years of progressively responsible experience in social service delivery.
• Experience with PC based word processing and spreadsheet software.
• Demonstrated ability to advocate, organize, problem-solve and provide results for the population(s) served.
• Able to work a varied schedule including evenings and weekends.
• Must have and maintain a valid Driver’s License in the state of residence.
• Must be able to communicate effectively verbally and in writing.
• Must be able to read and comprehend the English language.
• Fluency in Spanish a plus.

Responsibilities

• Resident Case Management: Ensure intake, annual assessments, ongoing referrals, and tailored service plans with periodic reassessment.
• Resident Programming And Engagement: Coordinate services with property team and implement resident engagement activities and measurable programs.
• Supporting Resident Basic Needs: Build partnerships for food access, rental and utility assistance, and eviction prevention.
• Administrative: Manage relationships and training expenses, complete reports, and produce monthly calendars and newsletters.
