• Valid driver's license in state of residence.
• Excellent verbal and written communication in English.
• Spanish language skills are a plus.

Responsibilities

• Resident Case Management: intake, annual assessments, referrals, and tailored service plans.
• Maintain records of resident interactions, goals, and outcomes.
• Follow up on lease violations to reduce evictions.
• Residential programming and engagement: collaborate with teams to meet service plans.
• Develop and implement ongoing resident engagement activities and partnerships with community providers.
• Collect data to measure resident and program outcomes.
