Job summary

Horizon Health & Rehabilitation Center in Las Vegas is seeking a full-time Social Service Assistant to join our team. You will complete the initial Social Service Assessment and the patient interview profile to begin discharge planning upon admission.

You will provide information to families and patients about Medicare, Medicaid, and other financial assistance programs, and work with Case Management to coordinate patient discharges.

Qualifications

• Must possess at minimum a High School Diploma or equivalent G.E.D.
• Social Work License preferred, not required.
• Must have experience in long term care or skilled nursing facility.
• Reliable transportation required.

Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission.
• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident.
• Work closely with Case Management to coordinate patient discharges.
