Fundamental in Albuquerque, NM is seeking a healthcare social services assistant to support discharge planning by conducting initial assessments and patient interviews. You will inform families about Medicare, Medicaid, and other financial assistance programs to help patients navigate costs.

Collaborate with Case Management to coordinate timely discharges and ensure smooth transitions from hospital to next care settings.

Qualifications

• Must possess at minimum a High School Diploma or equivalent G.E.D.
• Social Work License preferred, not required
• Must have experience in long term care or skilled nursing facility
• Reliable transportation required

Responsibilities

• Complete the initial Social Service Assessment and the initial patient interview profile to begin discharge planning upon admission
• Provide information to family and/or patients/residents relating to Medicare, Medicaid, and other financial assistance programs available to the patient/resident
• Work closely with Case Management to coordinate patient discharges
