Job summary

Llano Nursing and Rehabilitation seeks a licensed Social Worker to advocate for residents and lead the Social Service Department in coordination with the Administrator and applicable standards.

The role requires directing social services, coordinating with other departments, maintaining records, and supporting families through discharge planning while upholding residents’ rights and confidentiality.

Qualifications

• Licensed Social Worker in good standing with the state.
• Ability to interact positively with residents, families, and regulatory agencies.
• Willing to work long or irregular hours, including evenings and weekends.

Responsibilities

• Develop, implement, and direct all social services functions.
• Ensure an on-going Quality Assurance program and implement plans of action to correct deficiencies.
• Consult with and coordinate social services activities with other departments and implement changes as required.
• Ensure that necessary forms, reports, and progress notes are properly completed and descriptive.
• Participate in community planning, discharge assessments and discharge plans for residents and families.
• Coordinate social services with residents, families, staff, and community agencies.
• Provide information to families about available support agencies and financial assistance.
• Maintain resident confidentiality and uphold Residents’ Rights; treat residents with dignity and respect.
• Monitor residents’ emotional and social needs and ensure staff are informed.
• Performs other duties as assigned.
