Job summary

NHS Management, LLC seeks a Social Services Director to act as liaison for residents’ social needs and to plan, organize, and direct the Social Services Department in accordance with federal, state, and local standards.

The role requires a Bachelor’s in Social Work or Human Services, licensure as required, and healthcare experience. You will coordinate care plans, discharge planning, and collaboration with administration and nursing to improve quality of life for residents.

Qualifications

• Bachelor’s Degree in Social Work or Human Services required
• Must meet Federal and State licensure/certification as required
• Healthcare experience preferred
• Ability to communicate tactfully with residents, families, staff and agencies
• Must be 18+ and able to read/write English
• Able to perform essential job functions and physical/sensory requirements

Responsibilities

• Plan, develop, organize, implement, evaluate, supervise and direct social services programs.
• Ensure ongoing quality assurance plans and implement corrective actions.
• Coordinate social services with administration, medical and nursing staff.
• Maintain accurate forms, reports and progress notes.
• Participate in care planning, assessments and discharge planning.
• Coordinate with residents and families and community agencies.
• Provide information on support services and financial assistance.
• Maintain incident/accident reports related to behavior management.
• Maintain positive relationships with residents, families, staff and external resources.
• Assist in absence of family to meet residents’ needs.
