Covenant Living Communities and Services is seeking a Social Services Director in Evanston, IL. You will lead the social service function across multiple care levels, directing assessments, coordinating with the care team, and advocating for residents and families.

The ideal candidate will be a licensed Social Worker with an MSW preferred, with several years in long-term care and supervisory experience. Strong communication and organizational skills are essential, along with a commitment to

• License as a Social Worker in the State.
• Bachelor’s degree in Social Work; Master’s degree in Social Work preferred.
• Four years experience in working directly with individuals in a medically related field with long-term care experience.
• Two years in a supervisory capacity in the social work field.
• Knowledge of community resources.
• Excellent communication skills, verbal and written, in English, with tact and professionalism.
• Flexibility and adaptability.
• Good organizational skills with attention to detail and accuracy.
• Ability to follow through on assigned tasks and exercise good judgment and initiative.
• Ability to handle highly confidential information and establish trust.

Responsabilidades

• Assist as the facility representative with prospective residents, families and discharge planners for those needing skilled nursing care.
• Provide social assessment of residents at admission and identify psychosocial needs.
• Assist with orientation of residents to the skilled nursing facility and services.
• Record admission data accurately and address questions and family adjustments.
• Maintain documentation in the resident health record related to social services care plans.
• Ensure care plan documentation meets regulatory requirements.
• Participate in resident care planning meetings for department communication.
• Assist residents and families in selecting new physicians as needed.
• Communicate information to departments regarding admission or discharge as appropriate.
• Advocate for residents and families to ensure resident rights are upheld.
• Provide regular contact with families and act as liaison with the care team.
• Establish regular family support meetings for medically related social and emotional needs.
• Explain government and facility policies to new residents/families.
