Citadel Healthcare's Warren Park Health & Living Center in Chicago seeks a Social Services Designee/PRSC to address the social, emotional, and psychological needs of skilled nursing residents, promoting resident growth and well-being.

You will coordinate admissions and discharges with staff, guide residents and families through directives and guardianship choices, complete assessments, and ensure discharge planning is prepared at least 24 hours before discharge, in a compassionate, compliant

Qualifications

• Must read, write, and communicate in English.
• Strong verbal and written communication skills to perform required tasks.

Responsibilities

• Keeps informed about new and existing community resources.
• Communicates admissions and discharges with staff.
• Discusses choices for directives, financial power of attorney, guardianship, and surrogate decision with residents or their representatives.
• Documents discharge planning as an ongoing review and communicates with residents and families.
• Ensures home care needs are met with discharge planned 24 hours prior to discharge when date is known.
• Completes all required assessments and documentation promptly.
• All other duties as assigned.
