Job summary

The Center at Waterfront is seeking a full-time Social Services Designee to support our rehabilitation and skilled nursing program. You will plan, develop, implement and evaluate social services to meet residents' emotional and social needs in a supportive, compassionate setting.

Responsibilities include discharge planning, family interviews, and collaboration with the Administrator and care team to coordinate resources and support for residents' goals.

Qualifications

• Must possess, as a minimum, a Social Service Designee certificate.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating and directing social service programs.
• Participate in discharge planning; development and implementation of social care plans and resident programs.
• Interview resident/families as necessary and in a private setting.
• Perform administrative requirements, such as completing necessary forms, reports, etc. and submitting such to the Administrator as required.
• Involve the resident/family in planning social service programs when possible.
• Refer resident/families to appropriate social service agencies when the facility does not provide the services or needs of the resident.
• Provide information to resident/families as to medicare/medicaid, and other financial assistance programs available to the resident.
