Job summary

HCF is seeking a Resident Services Coordinator to ensure the emotional well-being and quality of life for residents served. You will coordinate social services, support admissions, and participate in PASSAR screens and care planning to promote overall well-being.

We value compassionate team players with a Bachelor's in Social Work or Human Services and at least one year of supervised social work experience in healthcare. A family-like caring environment and professional support are emphasized.

Qualifications

• Bachelor's degree in Social Work or Human Services with one year of supervised social work experience in a health care setting.

Responsibilities

• Possesses knowledge of policies and procedures for social service, admissions and PASSAR screens.
• Identifies psychosocial needs and interests of the resident upon admission and on-going.
• Completes sections of the MDS and develops care plans specific to each resident.
• Acts as a Resident Rights advocate, including staff education on resident rights and abuse.
• Implements discharge planning and documentation; provides referrals and coordination with outside agencies.
• Demonstrates ability to gather and utilize information within electronic health record systems.
• Participates in the QAPI process.
• Maintains professional competence.
