Job summary

HCF is seeking a Resident Services Coordinator/Social Worker to support residents' emotional well-being and quality of life. This full-time position focuses on social services, admissions, and discharge planning, with duties spanning care planning, rights advocacy, and coordination with outside agencies.

Qualifications include a PA; Bachelor's degree in Social Work or Human Services and one year of supervised social work experience in a health-care setting.

Qualifications

• Bachelor's degree in Social Work or Human Services with 1 year supervised social work experience in a health-care setting.
• Knowledge of policies and procedures for social services, admissions and PASSAR screens.
• Experience in a health-care setting is preferred.

Responsibilities

• Identify psychosocial needs and interests of residents upon admission and ongoing.
• Complete sections of the MDS and develop care plans specific to each resident.
• Advocate for resident rights and educate staff on rights and abuse.
• Assist in discharge planning and documentation.
• Provide services through referrals and communication with outside agencies.
• Utilize electronic health record systems to gather and use information.
• Participate in the QAPI process.
• Maintain professional competence.
