Job summary

Sea Mar Community Health Centers is seeking a Social Services Coordinator for Cannon House. The role focuses on assessing residents' medically-related social services needs, coordinating interdisciplinary care planning, and implementing services to support residents' rights and well-being.

The position requires a Bachelor’s degree in Social Work and preferably one year of geriatric-focused experience. Background checks, BLS/CPR, and TB testing are required.

Qualifications

• Bachelor’s degree in Social Work from a school accredited by the CSWE, or allied social science degree.
• One year of geriatric-centered program experience preferred.
• Background check from DSHS and Washington State Patrol; CPR/BLS within 90 days of hire.

Responsibilities

• Coordinate resident social services needs within the interdisciplinary team.
• Assist in development of social work policies and procedures for residents.
• Conduct psycho-social assessments and participate in discharge planning and care planning meetings.
• Provide crisis intervention and support to families; link residents to community resources.
