Job summary

Qualifications

• Social Services designee approved program or certificate at a Community College (for 99 beds or less).

Responsibilities

• Provide support to the Social Services Department and coordinate resident care.
• Assist with psychosocial assessments, Medi-Cal applications, transportation needs, and discharge planning.
• Coordinate care plan meetings with families at admission and quarterly thereafter.
• Assist with documentation and forms in accordance with policy and regulations.
• Help resolve grievances/concerns of residents and families.

Education
