Zusammenfassung

HumanGood is seeking a Social Services Coordinator to organize, implement and evaluate the Social Services program to meet residents' psycho-social needs and help them live independently in the community.

You will assess residents, link them to needed services, maintain records, and ensure privacy according to HIPAA standards while collaborating with the community team.

Schedule is Monday–Friday 9:00 AM to 4:00 PM. Pay ranges from $23.00 to $25.00 per hour, depending on experience.

Qualifikationen

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting.
• A Bachelor’s Degree in social work or related program is required.
• Social Services certification is preferred.

Description

Our Social Services Coordinators are passionate, organized, and compassionate towards residents. You would be responsible for organizing, implements and evaluates the Social Services program to meet the psycho-social needs of residents. This may include resident assessment, maintenance of resident records, assuring that residents are linked to the specific supportive services they need to continue living independently in the community.

• Monday - Friday at 9:00 AM - 4:00 PM

Pay Range: $23.00 - 25.00/hour, depending on experience

To be successful in the role, you would have:

• 1+ years of experience in case management required.
• 2+ years of social services experience, preferably in a senior care setting.
• A Bachelor’s Degree required in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred
