Job summary

Genacross Lutheran Services seeks a Resident Services Coordinator to help residents access resources and maintain independence across its communities. You will build relationships, assess needs, and connect residents with programs, benefits, and services.

The role emphasizes compassionate, person-centered support, documentation, and collaboration with community partners. Bachelor's degree required, with LISW/LSW preferred and a valid driver's license.

Qualifications

• Bachelor's degree in social work or related field preferred.
• LSW or LISW preferred.
• Valid driver's license required.

Responsibilities

• Build relationships with residents and serve as a trusted resource for questions, services, and community support.
• Assess resident needs and connect individuals with appropriate programs, benefits, and supportive services.
• Help residents navigate community resources and available assistance programs.
• Maintain accurate resident records, assessments, progress notes, and required documentation.
• Conduct annual apartment visits and update resident information as needed.
• Monitor resident needs and concerns and communicate significant issues to leadership.
• Complete required monthly reports and support ongoing program compliance.
• Report suspected abuse, neglect, or exploitation in accordance with applicable requirements.
• Build and maintain relationships with community organizations, faith communities, and service providers.
• Assist with PASSPORT Certification documentation and reporting.
• Serve as a Transition Coordinator for assigned Home Choice Program referrals.
• Participate in required training and professional development.
• Help ensure residents receive responsive, compassionate, and person-centered support.
