Qualifications

• Bachelor’s degree in Social Work, Gerontology, Psychology or Counseling; 5+ years of relevant work experience may be substituted.
• Ability to advocate, organize and problem-solve with residents.
• Strong communication in English; bilingual abilities valued.

Responsibilities

• Advocate for residents and address daily needs to support aging in place.
• Initiate 504 accommodation process with 504 Coordinator and staff.
• Develop and implement Supportive Services Plans and health-driven aging plans.
• Coordinate care with social services, hospitals, and providers to align resources.
• Provide case management and update plans as residents’ needs evolve.
• Build partnerships with community organizations and educate residents about services.
• Facilitate group meetings, workshops, and community engagement activities.
