Job summary

Akron Metropolitan Housing Authority is seeking a Service Coordinator for Elderly & Disabled to support the Resident Services team in Akron, OH. You will assess residents, arrange services, and build collaborations with community organizations to enhance programs and housing stability.

The role requires a Bachelor’s in social services with 3–5 years of relevant experience; LSW preferred. Proficiency in case management software and Microsoft Office is essential, with Tenmast knowledge desirable.

Qualifications

• Bachelor’s degree in social services or related field required; 3–5 years of experience with elderly/disabled population preferred.
• LSW preferred; valid Ohio driver’s license and insurability under AMHA policies.
• Proficient in Case Management Software and Microsoft Office; Tenmast desirable.

Responsibilities

• Interviews residents or conducts in-home visits to complete a needs assessment and arrange services.
• Develops case plans with residents to help elderly and disabled remain in housing and independent.
• Accurately enters case notes and data in case management software; maintains records for grants and reports.
• Promotes use of program services to enhance residents’ health and social functioning.
• Coordinates on-site programming and events; supports resident councils and trainings.
