Job summary

Astoria-Lake Wales is seeking a Social Services Assistant to support the Social Services Director in providing resident-focused care. You will help collect social histories, coordinate transportation, monitor mood and behaviors, and assist with care plans while maintaining confidentiality and regulatory compliance.

The role emphasizes teamwork with nursing and social work staff, adherence to safety protocols, and participation in CQI related activities. A high school diploma or GED is required.

Qualifications

• Must possess a high school diploma or GED.
• Thorough understanding of the principles of best Social Services practices.
• SSD certification preferred but not required.
• Previous experience in long-term care preferred but not required.

Responsibilities

• Assist the Social Services Director in operating the Social Services Department.
• Interview residents and families to obtain social history.
• Assist in arranging transportation and monitoring resident mood/behaviors.
• Develop and implement behavior management programs.
• Maintain transfer documentation and care plan records.
• Ensure confidentiality and resident rights protection at all times.
• Report incidents and adhere to facility policies and safety protocols.
