Job summary

American Medical Administrators MGMT Company is seeking a compassionate Social Services Director to lead resident and family support within our Mount Vernon facility. You will oversee psychosocial assessments, coordinate community resources, and guide the care team to ensure dignity and quality of life for residents.

The role requires a Bachelor’s degree in social work or related field and at least one year in a healthcare setting; strong communication, teamwork, and a patient-centered approach

Qualifications

• Bachelor’s degree in social work; OR
• Bachelor’s degree in a human-services related major (sociology, gerontology, psychology, rehabilitation counseling or special education) requirement
• One year in a healthcare setting working as a supervised social worker with individuals

Responsibilities

• Provide social services to residents and their families under the direction of the social services director
• Meet new residents on admission and complete an Initial Psychosocial Assessment
• Visit residents to follow up on concerns and assist with Medicaid/Medicare/SSI documentation
• Arrange items for residents, coordinate appointments with dental/eye/podiatry/audiology providers
• Support residents during times of loss and assist with gathering required documents
• Communicate effectively with residents, families and staff

Education

Bachelor's degree in social work; OR a related human-services major
