Reliant Care Management Company seeks a compassionate Social Services Director for our Mount Vernon, OH facility. You will oversee resident social services, coordinating with families and the care team to ensure quality support and resource access.

Responsibilities include admissions assessments, follow-up visits, coordinating with healthcare providers, assisting with Medicaid/Medicare/SSI documentation, and supporting residents during times of loss.

Formación

• Bachelor’s degree in social work or related human-services field.
• One year of healthcare setting experience as a supervised social worker.
• Ability to communicate effectively in writing and speech.

Responsabilidades

• Meet new residents on admission and complete an Initial Psychosocial Assessment.
• Visit residents to follow up on concerns and coordinate with care teams.
• May complete some MDS assessments under supervision of social services professional.
• Arrange needed items for residents and ensure adequate clothing.
• Schedule visits to dental, optometry, podiatry, and audiology services.
• Provide support in times of loss to residents and families.
• Assist residents in gathering documents for Medicaid, Medicare, SSI, etc.
• Perform other duties as directed by the Social Services Director.
