Job summary

Sullivan Park Care Center is seeking an Activity Assistant to help plan and conduct individual and group activities, ensuring a high level of resident engagement and well-being. You will work with staff, families and agencies to meet residents' needs under the Activity Director's guidance.

The role involves developing calendars, arranging transportation when needed, and assisting with assessments and action plans while maintaining a safe department environment.

Qualifications

• High school diploma or equivalent.
• Preferable one-year experience in a long term care facility.

Responsibilities

• Plan and conduct individual, small and large group activities.
• Assist in communications between staff, residents, families, support personnel and agencies.
• Develop monthly activity calendar and track attendance.
• Assist in discharge planning and activity care plans.
• Arrange transportation for residents when necessary.
• Maintain a clean, secure Activity Department.
