Job summary

Burbank Healthcare and Rehabilitation Center is seeking a Social Services Director to join our interdisciplinary team and supervise an assistant. The Director will foster policies and routines that maximize residents’ individuality, independence, and dignity, ensuring high levels of psychosocial well-being and quality of life.

Responsibilities cover policy review, advocacy, clinical oversight, and collaboration with administration to meet government requirements for social service documentation

Qualifications

• Knowledge of Long Term Care facilities.
• At least 2 years of Social Services experience.
• Understanding of resident rights and psychosocial care

Responsibilities

• Review facility policies and procedures as part of the interdisciplinary team to assure compliance with state and federal regulations.
• Participate in developing facility social work policies and quality improvement initiatives.
• Develop, maintain and utilize a listing of community resources useful to residents and families.
• Participate in quality assurance interdisciplinary team meetings.
• Meet government requirements for social service documentation.
• Document progress in meeting residents' psychosocial needs.
• Coordinate discharge planning and referrals for home care services when needed.
• Provide advocacy to protect resident rights and address abuse as required by law.
• Illuminate opportunities for resident self-advocacy and community involvement.
