Job summary

Villa At the Bay is seeking a Social Services Designee/PRSC to join their team. The role focuses on meeting the social, emotional, and psychological needs of skilled nursing residents and promoting their growth toward full potential.

The position involves discharge planning, resource coordination, and communicating with residents, families, and staff. The role requires awareness of regulatory standards and applicable laws while supporting compassionate, person-centered care.

Qualifications

• Must have a high school diploma or equivalent.
• Experience working with geriatric residents is preferred.
• Knowledge of Medicare, Medicaid, HMO and private insurance coverage.
• Knowledge of accrediting organization standards, IDPH regulations, state and federal regulations related to skilled care facilities.

Responsibilities

• Keeps informed about new and existing community resources.
• Communicates daily with staff regarding admissions and discharges to various units, facilities, hospitals, community and home.
• Discusses choices for advanced directives, financial power of attorney, guardianship, surrogate decision with resident or representative.
• Identifies and reports facility abuse per policy and applicable laws.
• Documents discharge planning and maintains ongoing resident/family communication to assess needs.
• Treats residents and families with respect, addressing psychosocial, cultural, ethnic and religious needs.
• Discusses advanced directives/POLST and end-of-life care options with residents and family.
• Coordinates with Case Manager on resident financial status and insurance.
• Ensures home care needs are met before discharge; targets arrangements 24 hours prior to discharge.
• Completes assessments and documentation promptly.
• Performs all other duties as assigned.
