The Advocate Healthcare Center of East Boston is seeking a Social Services Designee to support residents and families throughout their stay. You will assist with psychosocial assessments, discharge planning, adjustment support, and ensuring residents receive compassionate, coordinated services.

Qualifications

• Experience in Social Services or Human Services in healthcare or LTC, preferred.
• Bachelor’s degree in Social Work or related field preferred.
• Licensed Clinical Social Worker (LCSW) or a Bachelor’s degree in Social Work required.
• Minimum of 1 year of experience working in a Skilled Nursing Facility (SNF).
• Strong communication, empathy, and problem-solving skills.
• Ability to collaborate in a fast‑paced, resident‑centered environment.
• Understanding of resident rights, abuse prevention, and mandatory reporting requirements.
• Strong family engagement and conflict resolution skills.
• Ability to complete psychosocial assessments, including MDS Section D (PHQ-9) and Section Q (discharge planning).
• Competence in developing and updating person-centered care plans.
• Experience with behavioral interventions and trauma-informed care approaches.

Responsibilities

• Conduct psychosocial assessments for new admissions
• Provide support, counseling, and advocacy to residents and families
• Assist with discharge planning and community resource coordination
• Maintain documentation in compliance with state and federal regulations
• Participate in care plan meetings with an interdisciplinary team
