Job summary

Castle Healthcare seeks a Social Service Designee to plan, organize, and support the Social Services Department in accordance with federal, state, and local standards. The role ensures psychosocial needs are met for residents and supports the interdisciplinary team under the Administrator and Director of Social Services.

Qualifications

• Experience in social services or social work settings.
• Ability to document and maintain records per regulations.
• Strong collaboration with interdisciplinary teams.

Responsibilities

• Review and help set policies for resident care and quality of life.
• Develop and implement social work policies under DSS direction.
• Participate in quality assurance meetings with the DSS.
• Ensure compliance with government requirements for social service documentation.
• Document progress addressing psychosocial needs of residents.
• Assist in discharge planning and referrals to appropriate services.
• Coordinate psychosocial assessments and care plans with the team.
• Provide support to residents and families and address resident rights.
