Qualifications

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop and implement individualized treatment plans to improve mobility.
• Supervise PT assistants and training for staff as needed.
• Communicate progress with physicians, families, and nursing staff.
• Complete timely, accurate documentation per policy and payer requirements.
• Maintain safe treatment environment with infection control measures.
