• Assists in completion of care plans and psychosocial assessments.
• Coordinates care plan meetings with families.
• Coordinates room changes and roommate notifications.
• Conducts tours for potential residents and their family members.
• Reviews contracts to obtain prior approval for equipment as indicated.
• Completes records and documentation in accordance with company policy and state and federal guidelines.
• Back-Up for the Director of Social Services when out on PTO, FMLA or the Director of Admissions position is open.
• Protects patient information confidentiality and privacy.
