Job summary

Christian Care Communities Hopkinsville Health Center is seeking a Resident Service Coordinator to support residents in an affordable housing community. The role focuses on coordinating services, conducting assessments, and developing tailored plans to enhance residents’ wellbeing.

You will work with the property team to ensure services align with the resident services plan, engage residents through programs, and build partnerships with community organizations.

Qualifications

• Bachelor degree in social services or related field or equivalent experience.
• 2+ years of progressively responsible social service delivery experience.
• Must have and maintain a valid Driver’s License in the state of residence.
• Excellent verbal and written communication skills.

Responsibilities

• Resident Case Management: complete intake, assessments, referrals, and tailored service plans; reassess when services are unavailable.
• Maintain records of resident interactions, goals and outcomes.
• Follow up on lease violations to reduce evictions.
