Job summary

Outfield-Healthcare-Partners in Missouri seeks a friendly Activity Director to plan and implement a vibrant program for residents in a long-term care setting. You will lead engaging activities addressing social, intellectual, emotional and physical needs, while coordinating with care staff to support individualized care plans.

You will document assessments, complete required forms, and maintain monthly calendars.

Qualifications

• Certified Activity Director accreditation and state-approved training.
• Bachelor’s degree in recreation therapy or related field; licensed or registered.
• Minimum 1 year experience as Activity Director in long-term care.
• Two years of social/recreational program experience in the past five years.

Responsibilities

• Develops and delivers activity/recreational programming to promote the resident’s opportunities for engaging in normal life.
• Completes the MDS and CAA sections, attends and participates in Resident Care Plan Meetings.
• Conducts admissions interviews, documents activities, and writes care plans.
• Prepares monthly activity calendars and posts them in the center.
• Identifies activity preferences and health issues affecting participation; documents findings.
• Completes standardized assessment data and develops individualized care plans.
• Implements strategies to increase resident involvement in appropriate activities.
• Plans community activities involving staff, families and residents.
• Obtains physician approval for activities and maintains safety lists.
• Provides opportunities for group and individual leisure pursuits and encourages participation.
• Responsibilities may change or be assigned at any time.
