Job summary

Outfield Healthcare Partners is seeking an outgoing Activity Director to lead activity and leisure programs for residents in a long-term care setting. You will plan, organize, and implement activities designed to meet social, spiritual, intellectual, emotional, educational and physical needs, aligned with each resident’s care plan.

Qualifications

• Certified Activity Director accreditation per state requirements.
• Completion of state-approved Activity Director training course.
• Bachelor’s degree in recreation therapy or related field with license.
• Minimum 1 year as Activity Director in long-term care.

Responsibilities

• Develop and deliver activity programs to meet social, spiritual, educational and physical needs.
• Complete MDS activity sections and care plans; attend care plan meetings.
• Prepare monthly activity calendars and post for residents.
• Identify resident preferences and health issues affecting activity participation.
• Create individualized care plans based on identified needs and strengths.
• Encourage resident involvement in group and individual leisure pursuits.
