Job summary

Responsibilities include coordinating with families, leading interdisciplinary care plan meetings, documenting progress, and ensuring regulatory compliance.

Qualifications

• Licensed social worker per Nebraska requirements.
• Minimum of two years of social work experience in healthcare.
• Must complete Social Services Certification through The State of Nebraska.
• Ability to maintain regular attendance and complete training programs.
• Ability to meet local health regulations and pass a criminal background investigation.

Responsibilities

• Orient new residents to the long-term/sub-acute environment and assist with facility adjustment.
• Develop and implement individualized care plans including social histories and goals.
• Document progress notes and participate in interdisciplinary care plan meetings.
• Evaluate resident needs and revise care plans as needed.
• Assist residents and families in discharge and placement planning.
• Organize family groups and provide counseling as needed.
• Participate in education activities, staff meetings, and training programs.
• Promote positive relationships with residents, visitors, and regulators.
