Qualifications

• Bachelor’s in social work or related field required.
• 1 year supervised social work experience in healthcare preferred.
• Current State Clinical Social Work Licensure required.
• Maintain a working email address and phone number for employer communication.
• Ability to work cooperatively as a team member.
• Ability to communicate effectively with residents and families at all levels.
• Ability to react decisively in emergencies.
• Ability to maintain confidentiality.
• Ability to read, write and speak English.

Responsibilities

• Coordinate social service and discharge planning within the hospital system.
• Develop measurable plans and objectives to improve social services daily.
• Perform psychosocial assessments and reassess residents’ needs.
• Ensure resident safety and reduce health care errors through appropriate care planning.
• Involve residents and families in care planning and teaching.
• Provide resources and support to Social Service staff and clinical staff.
• Maintain rapport with medical staff and operate equipment safely.
• Promote residents’ rights and the Residents’ Bill of Rights.
