Job summary

Harborview Health Systems seeks a Licensed Social Worker to lead planning, organization, and evaluation of the facility’s Social Services Department. You will ensure regulatory compliance and coordinate with interdisciplinary teams to address residents’ social and emotional needs.

You will support admissions, transitions, and care conferences, advocate for residents, and help develop individualized care plans.

Qualifications

• Bachelor’s degree in Social Work (BSW) or in a human services field such as sociology, psychology, special education, or rehabilitation counseling.
• Minimum of 1 year supervised social work experience in a healthcare setting working directly with individuals.
• Knowledge of federal and state long-term care regulations.
• Strong interpersonal, communication, and problem-solving skills.
• Resident-first mindset with dignity, empathy, and respect.
• Must meet licensure and certification requirements in the state of practice.

Responsibilities

• Assist in planning, organization, implementation, and evaluation of the facility’s Social Services Department.
• Ensure social services are delivered in compliance with guidelines and Harborview's policies.
• Develop and monitor individualized care plans addressing residents' social and emotional needs.
• Provide ongoing support during admissions, transitions, and care conferences.
• Recommend equipment, services, or referrals for outpatient and home health support.
• Participate in policy development and improvements to care delivery.
• Organize and participate in in-service training for the social services team and staff.
• Stay up-to-date on regulatory changes and standards in long-term care social work.
• Advocate for residents and liaise among staff, families, and external agencies.
• Collaborate with interdisciplinary care teams for holistic care.
