Greenspring Village by Erickson Senior Living in Springfield, VA, is seeking a full-time resident-centered Social Worker to support our 62-resident Skilled Nursing neighborhood. You will provide social work services across skilled nursing, long-term rehab, assisted living, and memory care onsite, with one weekend day required.

Qualifications

• Minimum of 3 years' experience working with older adult populations.
• Minimum of 2 years experience in Skilled Nursing / Long Term Care.
• License: Social Worker License for Virginia preferred.
• Social Work Degree required, MSW preferred.
• Experience with EHR / EMR preferred.
• Proficiency with Microsoft Office, including Word and Outlook.

Responsibilities

• Completes psychosocial assessments, MDS, and other required documentation per policy and regulations.
• Coordinate the Social Work function from admission through discharge, including room changes and transition planning.
• Participate in care planning with a collaborative team to address resident preferences and needs.
• Provide bereavement support to residents, families, and staff.
• Monitor psychosocial and medically related social service needs and document in the medical record.
• Ensure compliance with Federal, State and local regulations for long-term care/assisted living.
• Identify and help resolve grievances and service issues with residents and families.
