Job summary

Oak Crest Village by Erickson Senior Living is seeking a Social Worker to provide social work services to residents and families within Continuing Care. The role focuses on coordinating care from admission to discharge and supporting transitions with dignity and respect.

The position is PRN/Flex to cover vacations, sick time, or absences during typical office hours. Compensation is $40.00 per hour, with benefits including 401k, on-site medical centers, education assistance, and growth

Qualifications

• Social Work degree required (BSW or MSW) or minimum education requirements per state licensure.
• Minimum of 3 years’ experience with older adult population in post-acute/long term care preferred.
• Knowledge of geriatric population and dementia.

Responsibilities

• Completing admission assessments, MDS, and other required documentation in accordance with State and Federal regulations
• Educating residents/families on advance directives and facilitating completion, signature, and tracking of documents
• Coordinating the Social Work function from admission to discharge to include room changes and transition planning
• Providing psychosocial support to residents and families to ensure a successful transition to Continuing Care
• Participating in care/service plan meetings with family members
• Providing bereavement support to residents, family members, and staff

Location: Oak Crest Village by Erickson Senior Living

Join our team as a Social Worker. In this role, you will be responsible for providing social work services to the residents and family members of Continuing Care.

Schedule: PRN/Flex arrangement to support the team with covering vacation, sick time, or medical leave absences from full-time team members during typical office hours.

Compensation: $40.00 hourly.
