Job summary

Garden Place HealthCare seeks a Licensed Social Worker (LICSW/MSW) to support residents and families, promoting morale and coordinating care plans from admission through discharge.

The role includes assessments, explaining Resident Rights, and assisting with Medicare/Medicaid applications, while coordinating with the interdisciplinary team and providing counseling for adjustment and grief.

Qualifications

• Master's Degree in Social Work, Psychology or Related Field is required.
• Licensed Social Worker (MA) licensure or LICSW preferred.
• 1-3 years of long-term care or related experience is preferred.

Responsibilities

• Lead by example to promote positive morale within the facility.
• Work with residents, families and team members to outline goals of stay from admission to discharge.
• Conduct psychological screenings and draft initial plan of care with necessary paperwork (MDS).
• Explain Resident Rights to all new admissions.
• Collaborate with families, Business Office and Medicare/Medicaid for financial assistance qualification.
• Coordinate interdisciplinary team meetings to plan care and address concerns; write reports.
• Provide counseling to residents and families on grief, loss, coping with transitions.
